Moshava Men’s camp (MMC) will play an integral role in the growth of and investment in Camp Moshava Wild Rose’s infrastructure. Separate from the camp’s annual campaign, all proceeds from 2021 MMC will go towards helping complete the build out of the new lower levels of the Mishpachot housing. Suggested minimum donation to attend is $360 per person although any sponsorship level you can give will go a long way as this critically necessary project will cost $100,000 per building/$25,000 per room. As a bastion of Wild Rose past, present and future, our goal as the 2021 MMC cohort is to raise $25,000. We already qualify as Mishpachot – building beautiful places for us and other families to stay when we visit camp is a natural fit. Looking forward to a yet another great time together!
  • In consideration of my use of the property, equipment, and facilities provided by Camp Moshava, to the fullest extent permissible by law, I expressly agree and contract, on behalf of myself, my heirs, executors, administrators, successors and assigns, that Camp Moshava and its insurers, employees, officers, directors, and associates, shall not be liable for any damages arising from personal injuries (including death) sustained by me, in, on, or about the premises, or as a result of the use of the equipment, property, or facilities, regardless of whether such injuries result, in whole or in part, from the negligence of Camp Moshava. By the execution of this agreement, I accept and assume full responsibility for any and all injuries, damages (both economic and non-economic), and losses of any type, which may occur to me, and I hereby fully and forever release and discharge Camp Moshava, its insurers, employees, officers, directors, and associates, from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated, resulting from or arising out the use of said equipment, property, and facilities. I expressly agree to indemnify and hold Camp Moshava harmless against any and all claims, demands, damages, rights of action, or causes of action, of any person or entity, that may arise from injuries or damages sustained by me. I agree to be solely responsible for my safety and my well-being. I understand that Camp Moshava does not provide supervision, instruction, or assistance for the use of the property, facilities, and equipment. I agree to comply with all rules imposed by Camp Moshava regarding the use of the property, facilities, and equipment. I agree to conduct myself in a controlled and reasonable manner at all times, and to refrain from using any property, equipment, or facilities in any manner inconsistent with its intended design and purpose. I understand and agree that Camp Moshava is not responsible for property that is lost, stolen, or damaged while in, on, or about the premises. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity. I understand while participating in this activity, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose. The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. It is agreed that any dispute or cause of action arising between the parties, whether out of this agreement or otherwise, can only be brought in the court or “Beth Din” of the Chicago Rabbinical Council.

    Additional waiver: The coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Camp Moshava has put in place preventative measures to reduce the spread of COVID-19; however, we cannot guarantee that you will not become infected with COVID-19. Further, attending the Fantasy Camp could increase your risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending the Fantasy Camp and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Fantasy Camp may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Moshava employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at the Fantasy Camp or participation in Fantasy Camp programming (“Claims”). On my behalf, l hereby release, covenant not to sue, discharge, and hold harmless Camp Moshava, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Moshava, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in the Fantasy Camp program.

    I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
